Part A (Assessment Criterion 2.1) The liver. Q1 Dissect a mammalian liver. Prepare labelled drawings to show the gross anatomy of the liver from both aspects. The liver is situated in the abdomen just below the diaphragm (Houston et al., 2016). It consists of 4 distinct lobes the left, right, caudate, and quadrate lobes shown in figure 1, and is the only organ that regenerates itself enabling a partial transplant to be able to have a functioning liver (Houston et al., 2016). Liver cells have large numbers of mitochondria and a large Golgi Apparatus to enable ATP for their energy needs (Houston et al., 2016). Microvilli is also needed to increase the surface area to absorb substances from the blood (Houston et al., 2016). The liver has a vital function in red blood cells formation, it synthesizes the haematinic principle from vitamin B12 which is stored in the liver (Houston et al., 2016). The liver has many functions processing, manufacturing and storage (Houston et al., 2016). Processing The liver makes glucose out of carbohydrates when the body is low on energy (Houston et al., 2016). The liver metabolizes fat this is essential when glucose is running out (Houston et al., 2016). Excess carbohydrates and proteins are converted into fatty acids then release into the blood stream for energy (Houston et al., 2016). The liver filters what we eat, what gets absorbed into our blood and excretes it as solid (feces) and liquid waste (urine) (Houston et al., 2016). The liver converts fat-soluble toxins into a water-soluble substance (Houston et al., 2016). This allows them to be released through the kidneys for elimination in urine, or into bile for elimination through the colon (Houston et al., 2016). The liver filters and detoxifies the blood coming from the digestive tract before it goes onto the urea which is then filtered out by the kidneys and flushed out the body (Houston et al., 2016). Manufacturing The livers lobules constantly produce bile and travels to the gall bladder shown in figure 1, for storage (Houston et al., 2016). Bile is important for breaking down fats threw the process of emulsification (Houston et al., 2016).The liver secrets three hormones Insulin-like growth factor 1 which stimulates cell growth, Erythropoietin which stimulates the creation of red blood cells by the red bone marrow and Angiotensin II which helps blood pressure control (Houston et al., 2016).When amino acids are absent from a person diet the liver produces proteins that are then secreted into the blood (Houston et al., 2016).The liver also produces plasma proteins (Houston et al., 2016). Storage The liver stores Vitamin A, B12, D, E and K until needed (Houston et al., 2016).The liver also stores essential minerals ion and copper (Houston et al., 2016).Energy is also stored as glycogen, which activates enzymes in the liver cells (Houston et al., 2016). When the bodies low on energy enzymes in the liver converts the glycogen to glucose a process called glycogenolysis, then releasing it into the blood stream (Houston et al., 2016). Sugars are passed into the capillary network and transported via the hepatic portal vein to the liver (Houston et al., 2016). The liver maintains a constant level of blood sugar by converting excess sugar to glycogen, so it can be stored (Houston et al., 2016). Amino acids are passed into the capillary network and transported via the hepatic portal vein to the liver (Houston et al., 2016). Metabolically active tissue need a large supply of amino acids for protein synthesis (Houston et al., 2016). Any excess amino acids received by the body can be converted in to glucose threw gluconeogenesis (Houston et al., 2016). Amino acids cant be stored, alternatively they are deaminated in the liver (Houston et al., 2016). Fats are absorbed as fatty acids and glycerol, these pass easily through the lipid biolayer of the plasma membrane active transport is not required for them (Houston et al., 2016). The majority of these molecules then recombine to make lipids which combine with proteins to make lipoproteins (Houston et al., 2016). Some excess cholesterol and phospholipids are eliminated in the bile (Houston et al., 2016). Q2 Having covered the gross anatomy, you are now ready to move towards lobule structure. Prepare a labelled diagram showing an overview of one liver lobule. This needs to identify all four fluid connections of the lobule. The hepatic artery, hepatic portal vein, hepatic central vein also known as the Inferior vena cava and the bile duct are the four fluid connections shown in figure 2. (Rolfe, 2018). The Hepatic Portal vein carries blood from the spleen, pancreas and intestines and it contributes to 75 of blood volume to the liver (Rolfe, 2018). This blood is rich in nutrients but relatively poor in oxygen content (Rolfe, 2018). Hepatic artery is the branch of the celiac trunk from the abdominal aorta and it contributes to 25 of blood volume entering the liver, this blood is well oxygenated but nutrient poor (Rolfe, 2018). Blood from the portal vein and the Hepatic artery mixes as it enters and travels through a liver lobule (Rolfe, 2018) shown in figure 2. Blood then Penetrates the lobule and is collected by the Inferior vena cava and returned to the heart (Rolfe, 2018). Bile comprises the exocrine secretory product of the liver (Rolfe, 2018). It is stored in the gallbladder (Rolfe, 2018). During digestion bile is released from the gall bladder threw the cystic duct to the duodenum of the small intestine (Rolfe, 2018). Blood flows within sinusoids from a portal vein to a central vein (Marieb, 2007) shown in figure 3. Bile flows within hepatic plates from the middle to bile ductules at the periphery of a lobule (Marieb, 2007) shown in figure 3. Secreted bile flows through tiny canals called the bile canaliculi these run between hepatocytes towards the bile duct branches in the portal triads (Marieb, 2007). This process is called enterohepatic circulation (Marieb, 2007). Blood and bile do not mix in the liver lobules as they travel in opposite directions (Marieb, 2007). Hepatocytes change cholesterol into bile acids (Marieb, 2007). These bile acids bind with more cholesterol, other lipids, electrolytes, water and the waste products from the destruction of old red blood cells known as bilirubin (Marieb, 2007).Every sinusoid passes through liver tissue comprising of two main cell types Kupffer cells and hepatocytes (Marieb, 2007).Kupffer cells are part of the sinusoid walls, they remove debris like bacteria and worn out red blood cells from the blood as it travels past (Marieb, 2007).Liver cells called hepatocytes is a chemical process that forms a thick epithelium that creates a functional barrier between two fluid compartments with distinct ionic compositions (Marieb, 2007). Part B (Assessment Criterion 2.2) Q3 How does the liver contribute to blood glucose homeostasis The regulation of glucose concentration in the blood shows how homeostasis is maintained by negative feedback (Toole and Toole, 2004). The cells of the islets of Langerhans reacts to a drop-in blood glucose levels by secreting a hormone called glucagon straight into the blood stream (Toole and Toole, 2004). Liver cells are the only cells that have the ability to bind to glucagon (Toole and Toole, 2004), causes liver cells to produce the enzyme phosphorylase (Toole and Toole, 2004).Phosphorylases purpose is to convert glycogen to glucose (Toole and Toole, 2004) by increasing the conversion of amino acids along with glycerol into glucose (Toole and Toole, 2004).Gluconeogenesis is to increase the amount of glucose in the blood and taking it back to its normal level (Toole and Toole, 2004).This increasing of blood glucose level makes the cells to minimalize the secretion of glucagon (Toole and Toole, 2004).Adrenaline is a hormone which can also increase blood glucose (Toole and Toole, 2004). When a human is stressed or excited the adrenal gland makes adrenaline (Toole and Toole, 2004). Adrenaline enables the breakdown of glycogen in the liver (Toole and Toole, 2004) this increasing the blood glucose level (Toole and Toole, 2004). If the glycogen supply in the liver becomes drained, cortisol is produced by the adrenal gland (Toole and Toole, 2004), enabling the liver to convert amino acids and glycerol into glucose (Toole and Toole, 2004). The negative feedback process takes effect only when blood glucose levels drop below the set point, then the insulin secretion lowers (Toole and Toole, 2004), resulting in a rise in blood glucose (Toole and Toole, 2004). It the same for when levels go above the set point, (Toole and Toole, 2004), glucagon secretion lowers (Toole and Toole, 2004). Provide diagrams and brief annotations to analyse this process and its consequences. In homeostasis blood glucose control is a negative feedback loop (Brooker, 2008) shown in figure 4. The liver is a vital store of glycogen (Brooker, 2008). The liver can release Glycogen and break it down back into glucose this enabling the correction of blood sugar levels (Brooker, 2008) shown in figure 4. When glucose levels are too high the liver can absorb the glucose and convert it into glucagon for storage (Brooker, 2008). Once your glycogen stores are full, any excess carbohydrates or glucose are converted to glycogen and stored as fat or used in cell respiration for energy (ATP) shown in figure 5. A consequence of this would be obesity and possible diabetes (Brooker, 2008). Q5 The body cannot store amino acids. To maintain homeostasis, the liver therefore needs to eliminate excess amino acids. Amino acids are proteins which are not stored in the body like carbohydrates and lipids therefore excess amino acids are destroyed by the liver cells in a process called deamination (Kang, 2012).Deamination removes amino groups from the amino acid to make ammonia (Kang, 2012) shown in figure 6.Threw enzyme catalysed reactions ammonia is mixed with carbon dioxide which makes urea (Kang, 2012). Urea then travels through the blood to the kidneys, then travelling out in the urine. Where as amination is the process of adding an amine group (Kang, 2012). Transamination is the swapping of only the amine group of the amino acid for a keto-acid group (Kang, 2012) shown in figure 7. Lobules in the liver filter and process chemicals, alcohol being one, but they only have limited regeneration properties (Whitney and Rolfes, 2013). Therefore, if the cells have to keep dividing and replenish constantly and rapidly eventually it gets worn out and too damaged to be able to repair itself resulting in fatty cells on the liver (Whitney and Rolfes, 2013). Alcohol upsets our water balance, it blocks the pituitary gland from releasing ADH (Whitney and Rolfes, 2013). Alcohol is metabolized by a two-step enzyme system called the alcohol dehydrogenase (ADH) pathway (Whitney and Rolfes, 2013) shown in figure 8. Firstly, the enzyme ADH converts alcohol to acetaldehyde generating NAPH H (Whitney and Rolfes, 2013). Acetaldehyde is toxic and requires immediate metabolization to something nontoxic (Whitney and Rolfes, 2013). However, if acetaldehyde builds up, some will pass from the liver into the blood (Whitney and Rolfes, 2013). Next in the ADH pathway is catalyzed by the enzyme acetaldehyde dehydrogenase (ALDH) (Whitney and Rolfes, 2013). This converts acetaldehyde to acetic acid (Whitney and Rolfes, 2013). The second pathway is the microsomal ethanol oxidizing system (MEOS), shown in figure 8, which is used when large volumes of alcohol are consumed (Whitney and Rolfes, 2013). It helps prevent blood alcohol concentration from getting dangerous high (Whitney and Rolfes, 2013).
